Position Overview:

Schedule: Thu, 3pm-11pm, Fri, 7am-11pm, Sat 9am-11pm
Compensation: $39.50 / hour
The Advanced Practical Nurse II delivers essential nursing services and daily healthcare support within a medically intensive framework, tailored to cater to a distinctive and specialized demographic diagnosed with brain injuries, developmental delays, or intellectual disabilities, who present complex medical requirements.

Responsibilities:

The Advanced Practical Nurse II takes on comprehensive oversight of the program during the assigned shift as the lead staff/nurse. This role guarantees that care is administered with respect, dignity, and high quality, while also promoting individual choices.

Nursing Care:

Implements individualized treatment care plans meticulously in alignment with medical directives.

Assists with daily living activities as per each individual's plan, including but not limited to bathing, dressing, grooming, oral hygiene, foot care, and diabetes management.

Responsible for fulfilling all skilled nursing care requirements of the individual, including but not limited to wound management, GT/JT care, tracheostomy care, and nutritional support.

Observes and monitors clients for any changes in condition and promptly reports all concerns to the Program Director, Senior Program Director (if necessary), and Program Nurse.

Ensures that all care is executed following best practices, while maintaining the dignity and choices of the individual.

Ensures that daily living activities are completed, and that assistive devices are functional and readily available for use.

Supervision:

Provides clinical supervision to Certified Nursing Assistants during designated shifts, offering immediate training as needed.

Ensures that care is delivered in a timely and precise manner, meeting all Individual Support Plan (ISP) objectives and addressing the holistic needs of the person, not solely their medical requirements.

Communication:

Engages in communication and meetings with the RN Manager as required.

Maintains effective communication with primary care providers, family members, guardians, and other significant relationships.

Communicates proficiently with fellow Vinfen employees, external agencies, community organizations, and neighbors.

Documentation:

Records nursing care and treatments as prescribed by the primary care provider in accordance with Vinfen policies and procedures.

Documents medication administration following Vinfen policies and adhering to MAP standards.

Ensures all documentation is thorough and accurate, including but not limited to shift notes, daily communications, treatment protocol data, medication administration records, and the controlled substance inventory for each individual.

This includes reviewing the documentation completed by CNAs and direct care staff.

Environment:

Ensures the environment is safe, clean, comfortable, and welcoming for individuals, families, guardians, and other significant visitors.

Ensures the environment complies with all regulatory standards.

Promotes an atmosphere conducive to the active participation of clients in their residence.

Respects and honors the preferences and choices of clients.

Qualifications:

Valid Licensed Practical Nurse credentials in Massachusetts are required.

A high school diploma or equivalent is required; an Associate's degree is preferred.

One to two years of supervised practical experience, including internships, in a clinical or rehabilitation setting is beneficial but not mandatory.

Experience working with individuals with developmental and/or psychiatric disabilities and medical illnesses, as well as other co-occurring disorders, is preferred.

Preference will be given to bilingual/bicultural applicants and those with lived experience.

A strong commitment to the rights and abilities of individuals with complex health issues is essential.

Sensitivity to cultural, religious, ethnic, age, gender, and disability issues is required.

Must be at least 21 years of age, possess a valid driver's license for a minimum of 6 months, and have access to a reliable, insured vehicle.

Must be able to successfully pass background checks, including CORI, education, employment verification, references, driving history, and credentialing.
