We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality homecare services to pediatric and adult patients in Portsmouth, OH.

About the Role

As an LPN Homecare Specialist, you will be responsible for utilizing the nursing process to assess, plan, implement, and evaluate patient care. You will work closely with our team to provide skilled nursing care and services in accordance with prescribed orders.

Responsibilities
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
  • Utilize the nursing process to develop and implement individualized care plans.
Qualifications

To be successful in this role, you will need to have:

  • A current LPN/LVN license in the state of Ohio.
  • A current TB or Chest X-Ray.
  • A current BLS card.
  • One year prior professional nursing experience preferred.
