- Continuity of Care: Manage follow-up care, triage patient concerns, and facilitate timely referrals to ensure seamless healthcare delivery.
- Documentation: Maintain precise and timely records of patient interactions and treatment outcomes in the electronic medical record system.
- Collaborative Care: Work alongside physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
- Professional Development: Stay informed about advancements in medical practices and participate in quality improvement initiatives to enhance patient outcomes.
- Master's degree from an accredited Nurse Practitioner program.
- 1-2 years of experience in an outpatient care environment is preferred.
- Two years of experience in a rehabilitation setting is preferred.
- Current Nurse Practitioner license in New York State.
- Basic Life Support (BLS) Certification is required within 90 days of employment.
- In-depth knowledge of medical terminology and clinical procedures.
- Strong clinical assessment and diagnostic skills.
- Proficiency in electronic medical records and healthcare technologies.
- Excellent critical thinking, communication, and problem-solving abilities.
