About the Role:

We are seeking a skilled Pharmacist to join our team in a part-time capacity. As a key member of our pharmacy team, you will play a vital role in providing exceptional patient care and ensuring the safe and effective use of medications.

Responsibilities:

  • Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
  • Maintain accurate patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
  • Ensure laboratory tests are ordered, evaluated, and discussed with the healthcare team as necessary.
  • Collaborate with the healthcare team to provide clinical and technical support.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of medications.
  • Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
  • Inform the Pharmacy Manager of medication-related errors.
  • Assist facility/agency staff with procedures related to safe and effective methods of procuring, maintaining, administering, and disposing of medications.

Qualifications:

  • Bachelor's Degree in Pharmacy; PharmD preferred.
  • One year of experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
  • Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
