Job Summary

The Moments is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our care team as a Care Team Leader. This is a dynamic role that requires strong leadership and communication skills, as well as a passion for delivering high-quality care to seniors with dementia and other age-related conditions.

Key Responsibilities
  • Lead and direct the care team to ensure the health and safety of all residents are met.
  • Collaborate with residents, families, physicians, and Moments staff to create a person-centered plan of care that enhances the life and care of the resident.
  • Develop and implement care plans that address the unique needs and preferences of each resident.
  • Provide direct care to residents as needed, including administering medications, performing vital signs, and assisting with daily living activities.
  • Communicate effectively with residents, family members, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices in senior care and dementia management.
  • Contribute to a culture of compassion, respect, and dignity in the workplace.
Requirements
  • Current licensure as an LPN in the State of Minnesota.
  • Experience working in long-term care or senior care settings.
  • Strong leadership and communication skills.
  • Ability to work effectively in a fast-paced environment.
  • Basic Life Support (BLS) certification (preferred).
