Job SummaryWe are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct home risk assessments, medical history reviews, and provide health education to patients in their homes. This is a flexible, part-time opportunity that allows you to work independently and make a meaningful impact on the lives of our patients.
Responsibilities- Conduct home risk assessments and medical history reviews
- Provide health education and guidance to patients
- Collaborate with primary care providers and other healthcare professionals to ensure seamless care coordination
- Work independently in a variety of settings, including patients' homes, skilled facilities, and telehealth
- Masters degree in Nursing or equivalent experience
- Current RN and NP licensure in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
