About the Position

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment provider. In this role, you will conduct comprehensive health assessments, review medical history, and provide education to patients and their families.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Provide health education and counseling to patients and their families
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure comprehensive care
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth visits
Requirements
  • Master's degree in nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records

Travel Requirements

Ability to travel to patient locations, valid state driver's license, and adequate automobile insurance coverage required.
