Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families. You will work collaboratively with our team of clinicians, case managers, and social workers to ensure seamless care coordination.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Develop and implement care plans to improve patient outcomes
  • Provide education to patients and their families on health management and disease prevention
  • Collaborate with primary care physicians and other healthcare providers to ensure comprehensive care
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth visits
Requirements
  • Masters degree in nursing or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in state of practice, including prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
