Role Overview
The Oncology Care Coordinator plays a crucial role in patient care by managing oncology patient inquiries related to treatment, surgical procedures, and appointment scheduling.
Key Responsibilities
- Monitor and manage patient symptoms effectively.
- Oversee complex patient care to reduce emergency department visits and hospital readmissions.
- Identify and address barriers to care to ensure optimal patient outcomes.
- Facilitate effective communication among the multidisciplinary cancer care team to ensure seamless care delivery.
Qualifications
