Work Schedule: Monday to Friday, 8 AM to 5 PM
Incentive: Up to $2,500 hiring bonus for eligible candidates
Position Overview:
- Facilitates patient readiness for consultations by executing the Advanced Standard room process, which includes documenting the primary concern, measuring vital signs, assessing medical history and allergies, conducting a comprehensive medication review, and utilizing the Encounter Guide to address any outstanding care requirements such as fall risk assessments, mental health screenings, and preventive health checks.
- Provides assistance with necessary in-office evaluations, including but not limited to, phlebotomy, mental health assessments, electrocardiograms, oxygen saturation studies, and point-of-care diagnostics.
- Administers immunizations and other injections in accordance with professional guidelines.
- Coordinates follow-up appointments as per clinic protocols.
- Works collaboratively with healthcare providers and the clinical team to oversee and implement the care strategy.
Qualifications:
Education: Diploma in Practical Nursing or Registered Nursing
Licensure: Valid LPN or RN license
Preferred Experience: At least 6 months of nursing experience, ideally in a primary care setting.
