Key Responsibilities
  • Provide case management services to clients, including assessment, planning, and coordination of care.
  • Develop and maintain relationships with clients, families, and community resources to ensure comprehensive support.
  • Collaborate with multidisciplinary teams to provide counseling, education, and advocacy services.
  • Conduct outreach and engagement activities to connect with clients in the community.
  • Document client progress and maintain accurate records.
