Job Title: RN - Client Care Coordinator

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Client Care Coordinator. This role involves traveling to clients' homes to perform assessments and supervisory visits.

Key Responsibilities:
  • Conduct in-home assessments of clients
  • Perform caregiver supervisory visits
  • Maintain accurate and timely documentation
  • Communicate effectively with clients, families, caregivers, and other healthcare professionals
Qualifications:
  • Valid Registered Nurse (RN) license (Bachelors or Associates)
  • Strong interpersonal and communication skills
  • Ability to work independently and manage time effectively
  • Reliable transportation and willingness to travel
