Job Title: Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Mental Health Partners. As a Care Coordinator, you will play a vital role in providing essential support and guidance to our clients facing complex mental health, substance use, and environmental challenges.

Key Responsibilities:
  • Manage a caseload of clients and their diverse needs, providing individualized support and guidance.
  • Collaborate with clients, families, and healthcare providers to develop and implement comprehensive treatment plans.
  • Provide education and support to clients on mental health conditions, treatment options, and wellness strategies.
  • Facilitate connections to community resources, services, and support groups to promote client stability and well-being.
  • Document client progress, outcomes, and challenges in a timely and accurate manner.
Requirements:
  • Bachelor's Degree in Psychology, Social Work, Counseling, or a related field.
  • Experience working with adults in an outpatient setting, with a focus on mental health and substance use issues.
  • Knowledge of clinical modalities, including Motivational Interviewing (MI) and Dialectical Behavior Therapy (DBT).
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work collaboratively as part of a multidisciplinary team.
