Job Title: Transitional Living Specialist

Methodist Home for Children is seeking a dedicated and compassionate Transitional Living Specialist to join our team at our transitional living home for male youth in New Bern, NC.

About the Role

We are looking for a skilled professional to provide comprehensive case management and support to youth in their transition to independent living. As a Transitional Living Specialist, you will work closely with our team to facilitate vocational training, education, and career goals, while also coordinating Child and Family Team (CFT) meetings and monitoring resident progress.

Responsibilities
  • Provide individualized case management to support youth in their transition to independent living.
  • Facilitate vocational training and assist in setting education and career goals.
  • Coordinate CFT meetings and ensure collaboration between all stakeholders.
  • Monitor and document resident progress, creating a tailored transition plan during their residential placement.
  • Serve as a mentor and community advocate, connecting residents with local resources for vocational opportunities.
  • Guide youth through aftercare services for up to six months post-placement, providing essential support as they transition to independence.
Requirements
  • Must be at least 21 years old.
  • Hold a Bachelor's Degree (preferably in a human services field) with at least two years of experience working with a similar population.
  • Possess a valid driver's license and a clean driving record.
  • Ability to work a flexible schedule, including evenings, weekends, holidays, and extended shifts when needed.
