Working Conditions: This role requires working in a typical office setting and from home or other community locations as established by flexible work agreements, completing paperwork and documentation, and meeting clients in various settings. Reliable transportation is essential, with occasional long drives required. The position demands sitting, standing, bending, and reaching, as well as manual dexterity for operating standard office equipment. Normal hearing and vision are required, along with the ability to visit multiple families daily.

Job Summary: The Family Services Case Manager plays a vital role in ensuring the safety and well-being of children and families in foster care. Key responsibilities include facilitating Family Support Team meetings, developing individualized care plans, coordinating services across settings, and documenting activities and services. This role promotes effective utilization of support services to achieve permanency for children and families.

Key Responsibilities

  • Maintains contractual and agency policies, ensuring seamless delivery of services to children and their families.
  • Manages a caseload of 12-15 children, depending on program capacity and agency needs, with potential temporary increases due to staff absences or vacancies.
  • Provides full case management services to children and families in the MO foster and adoptive care system from assignment to case closure or transfer.
  • Documents all activities and maintains accurate data entry in FACES and CSWIS in a timely manner.
  • Has face-to-face contact with assigned children and families at least monthly, with telephone contact with support providers regularly to review progress and receive reports.
  • Evaluates and reports the impact of services to team members and judiciary, attends court hearings, and schedules, coordinates, and facilitates FST meetings within prescribed time frames.
  • Participates in the FCCM consortium on-call schedule as EOC for PCHAS on a rotating basis.

Requirements: A bachelor's degree in social work, psychology, counseling, education, criminal justice, or a related human services field is required, with a master's degree preferred. Two years of relevant case management experience are also necessary.

Essential Skills:

  • Represents Presbyterian Children's Homes and Services to the professional community, demonstrating strong verbal and written communication skills and representing the agency well. Builds and maintains positive relationships with Children's Division personnel, placement and treatment providers, schools, and agency partners.
  • Works a flexible schedule, as approved by the FCCM Supervisor, to meet the needs of families, often outside normal working hours. Plans, arranges, and provides crisis intervention services as needed, maintaining confidentiality and HIPAA standards.
  • Operates computers using agency-approved software programs and other office machines, such as copiers, calculators, telephones, etc. Navigates multiple data entry systems efficiently.
  • Analyzes variables and chooses the most effective course of intervention, handling novel and diverse work problems daily. Resolves problems, handles conflict, and makes effective decisions under pressure.

Other Requirements: A valid driver's license and risk-free driving record are required. Initial and periodic screenings for child abuse/neglect, criminal history, sexual offender, and childcare/elder care disqualification lists are necessary, conducted by the Missouri Department of Health and Senior Services utilizing the Family Care Safety Registry. Fingerprints may be submitted for review by the Missouri State Highway Patrol and Federal Bureau of Investigation (FBI). A pre-employment drug screen and physical examination are required, with no evidence of communicable disease. American Red Cross first aid and CPR certification are mandatory at all times.
