Job Summary

This position is responsible for ensuring the health and safety of all clients, working closely with the Director of Nursing to plan, coordinate, and manage the home care service. The RN will act within the Comprehensive Home Care regulations and be responsible for the overall direction, coordination, and monitoring of home care service delivery.

Qualifications
  • Must be licensed with the State of Minnesota as an RN and maintain current and unencumbered licensure.
  • Must have a valid driver's license if driving is required.
Specialized Knowledge and Abilities

This position requires the incumbent to have:

  • A sensitivity and aptitude for working with the elderly.
  • The skills to communicate effectively with those you supervise, clients, family members, and the Director of Nursing.
  • Leadership/supervisory skills.
  • The ability to prioritize and organize work effectively and efficiently.
  • The ability to work irregular hours, occasional weekends, and evenings.
Essential Job Functions and Tasks

Job Function:

  • Work within Home Care program guidelines, including completing essential paperwork and complying with state regulations.

Tasks:

  • Ensure the facility is in compliance with federal, state, and local standards and regulations, and be available for license surveys.
  • Maintain up-to-date knowledge of current trends in Home Care.
  • Work to maintain privacy and confidentiality of client, client's records, and client living environments.
  • Be willing to carry a cell phone and be available for staffing and notifications.
  • Communicate and document client data to staff as needed.
  • Assure clients maintain residency requirements, monitor, and recommend alternative placement if necessary.
  • Review client service agreements with RN as needed.
  • Monitor and maintain client charts and nursing notes.
  • Communicate to physicians any changes in client's needs or conditions as needed.
  • Monitor and follow through with ordering medications and all medication renewals, as well as any necessary follow-up with physicians regarding medications.
  • Fills weekly pillboxes for medication administration by unlicensed staff and checks new medications from the pharmacy.
  • Communicate in staff communication book any information that staff needs to be aware of.
  • Assist with compliance in meeting regulations and be available for state licensure surveys.
