Job Summary

The ideal candidate will be a Board Certified Family Nurse Practitioner with experience in urgent care or ambulatory settings. New grad Nurse Practitioners are also welcome to apply. The successful candidate will work with our team to deliver exceptional personalized care to patients in their homes.

Responsibilities
  • Perform annual comprehensive health assessments in patients' homes
  • Work autonomously to see Medicare age patients
  • Travel up to 120 miles per day, with mileage reimbursement
  • Collaborate with our team to develop tailored lifestyle improvement plans
Requirements
  • Board Certified Family Nurse Practitioner or equivalent
  • Experience in urgent care or ambulatory settings
  • Ability to work autonomously and create own schedule
