At Fairview Health Services, we are seeking a highly skilled and experienced Nurse Practitioner to lead our Care Map program. This is a unique opportunity to work in partnership with our operations leaders to provide strategic direction and leadership for our Care Map program services across the system.
Job Responsibilities:- Develop and implement clinical protocols and tools consistent with best-known practices with our CQVC team.
- Communicate, educate, and assure compliance with the use of these protocols and tools.
- Partner with other Medical Directors and our broader leadership team to support our strategic priorities and change initiatives.
- Consistently seek out opportunities to expand knowledge of best practices and new developments within related areas and incorporate this knowledge into the operation of the program.
- Understand evidence and build evidence-based protocols with our teams.
- Utilize data to make decisions and help prioritize our Care Map program priorities.
- Understand and collaborate with clinical operations on clinical workflows to support our Care Maps.
- Drive Care Map engagement with our clinical teams, as well as medical, clinical, and operational leaders.
- Graduate of a school of nursing and accredited Nurse Practitioner training program.
- BC/BE through the AANP or ANCC.
- Licensed or ability to obtain APRN license with the Minnesota Board of Nursing.
- Ability to obtain and/or maintain DEA certification for the State of Minnesota.
- Meet threshold criteria/qualifications for Credentialing and Privileges.
- 3 years of clinical experience.
