Job Summary

Key Responsibilities
  • Assist residents and families with transition and adjustments to placement in our facility
  • Provide input on social, psychological, emotional needs of residents and families to the interdisciplinary team
  • Develop and implement social services that meet the physical, mental, and psychosocial needs of residents
  • Evaluate resident's response to therapy and write quarterly progress notes
  • Collaborate with the multidisciplinary team to develop a plan based on the assessed needs of the resident
  • Ensure residents have required evaluations for visual, audio, mental health, and dental needs
  • Arrange residents' appointments, including transportation
  • Support the activities and recreational program in a manner that is safe and sensitive to the rights of each resident
  • Assist residents with finances as necessary, ensuring accuracy of documentation
  • Ensure patients receive their mail
Requirements
  • High school diploma and/or GED required
  • AA or bachelor's degree preferred
  • Computer experience and database management required
  • Previous experience as a Social Worker preferred
  • Previous experience with geriatric population or long-term care setting strongly preferred
