Job Summary

This is a challenging and rewarding role that requires a high level of expertise and experience in working with individuals with intellectual disabilities. As an Assistant Manager Day Services, you will be responsible for coordinating and overseeing the habilitation services for individuals who have intellectual disabilities.

Key Responsibilities
  • Coordinate and oversee the habilitation services for individuals with intellectual disabilities
  • Plan, implement, and review program plans and participate in daily activities for individuals assigned to your caseload
  • Provide mentoring, support, and in-classroom supervision to Direct Support Professionals (DSPs) in providing meaningful and varied activities for the program day
  • Act as liaison between home and program to communicate any needs for program
  • Organize, conduct, and write team meetings as needed
  • Complete monthly notes in Medisked and submit according to OPWDD regulations
  • Run Medisked reports as required to ensure that daily data is completed within the proper time frame
  • Attend scheduled Coordinator meetings
  • Work with Care Manager to schedule annual and semi-annual meetings with program/care givers to ensure necessary assessments are completed and that meeting space is available
  • Write the Staff Action Plan (SAP) or Habilitation Plan in Medisked, based on the needs established in the individual's Life Plan
  • Complete all of the necessary annual/semi-annual documentation (Personal Page, Supports, etc) and forward all meeting documentation to Day Services Information Specialist (DSIS)
  • Review finalized SAP and supports with staff to ensure proper implementation
  • Ensure that completed SAP/supports are distributed prior to effective dates (to residences where necessary for PWW IRA)
  • Ensure that individual's information is correct and update records in Medisked as needed
  • Participate with daily program and community integration activities
  • Assist individuals who have toileting needs
  • Assist with transferring or lifting of the individuals
  • Perform AMAP and SCIP responsibilities as needed
Requirements
  • Bachelor's degree and a minimum of 1 year experience working with the intellectually disabled, or High School Diploma/GED plus a minimum of 2 years experience working with the intellectually disabled population and demonstrated excellent oral and written communication skills
  • Readiness to learn and utilize relevant agency computer applications
