Job Summary

We are seeking a compassionate and dedicated Registered Nurse to join our Weekend Care Team as a Home Health Nurse. As a key member of our team, you will provide exceptional care to our patients in their homes, working closely with our Clinical Manager to ensure seamless coordination of care.

Responsibilities
  • Provide direct patient care as needed, with appropriate follow-up and documentation of care for agency clients in accordance with Quality Standards.
  • Participate in office duties as required, including scheduling, coordination of care, and referral processing.
  • Collaborate with staff to ensure the bridging of information from the weekend to weekday to facilitate appropriate management of patient care issues.
  • Contribute to clinical operational process and development, ensuring adequate skills and adherence with agency policies and procedures.
  • Assist with on-call as needed.
Performance Responsibilities and Standards
  • Ensure quality and safe delivery of home health services.
  • Comply with Nascentia Health policies and procedures and quality initiatives.
  • Participate in personal growth and development, including educational in-services and meetings.
  • Promote continuity of care for referred clients.
  • Promote quality assurance as it relates to home care planning.
Requirements
  • Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.
  • Current NY State driver's license, automobile insurance, and a vehicle which can be used for work.
  • Weekend schedule (8am-4:30pm) for every Saturday and Sunday preferred; rotating weekends available - minimum two weekends a month required.
Physical Requirements
  • Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
  • Ability to perform tasks involving physical activity, including pushing/pulling, lifting/carrying, and walking/driving in varied weather conditions.
