Job Overview:


Under the guidance of supervisory personnel and with a thorough understanding of patient characteristics such as age, gender, and other relevant factors, this role involves overseeing or managing the medication therapy of hospitalized or outpatient individuals as assigned. The specialist collaborates with medical staff to enhance patient care and treatment, provides medication information, and offers pharmacokinetic dosing recommendations. Responsibilities include adjusting dosages, ordering laboratory tests, and administering medications in accordance with established protocols; overseeing medication distribution; and dispensing, compounding, and distributing pharmaceutical products.

This position also entails providing direction to pharmacy support staff and performing additional duties as necessary.

Key Responsibilities:
Adheres to Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and relevant state, federal, and local regulations.
Engages in sterile and non-sterile compounding.
In partnership with the healthcare team, formulates and executes patient-specific strategies to optimize medication therapy.
Monitors medication therapy to ensure optimal therapeutic outcomes.
Manages and refines medication therapy based on data interpretation, assessments, and evaluations of patient results.

Follows hospital-approved collaborative pharmacokinetic drug therapy management protocols (e.g., adjusting drug dosages, ordering lab tests, assessing therapeutic effectiveness, etc.).

Conducts interviews with patients and their families or designated caregivers to gather information regarding medication usage, allergies, and sensitivities.

Integrates this information, along with relevant data from other sources, into the patient's medication management plan.
Documents information accurately in the patient's medical record or other electronic systems.

Provides guidance to patients and their families or designated caregivers regarding medication, usage instructions, storage requirements, medication coordination with diet, compliance importance, and safety precautions.

Advises on the use of medication-related devices.
Offers pharmacokinetic dosing guidance to medical staff.
Orders and modifies medication dosages, requests laboratory tests, and administers medications under established protocols in accordance with policies and procedures.
Monitors the accuracy and effectiveness of the hospital's medication distribution system.
Reviews all medication orders for appropriateness and compliance with the drug formulary.
Clarifies orders and intervenes when necessary.
Verifies the accuracy of medication preparation and dispensing by pharmacy staff.
Provides medical personnel with essential medication information.
Identifies potential medication safety concerns and actively works to prevent medication errors.

Reviews patient medical records to identify adverse drug events and collaborates with medical personnel to develop treatment plans in response to these events.

Assesses adverse drug events to determine their severity, likelihood, and preventability for further analysis and review.
Reviews patient medical records to ensure proper medication delivery and administration.
Ensures the appropriate storage and security of medications.
Participates in investigational drug studies as required.
Engages in problem identification and resolution, consulting with supervisors as needed.
Supports a collaborative Labor-Management Partnership environment.
Reports discrepancies to the appropriate medical and/or administrative personnel.
Develops and tests requirements and specifications for protocols, computer programs, pharmacokinetic dosing, and other pharmaceutical care applications.
Participates in assigned educational and training programs.
Creates and maintains educational curricula and conducts training programs for pharmacy staff, other medical personnel, and patients.
Actively participates in rounds, hospital committees, utilization reviews, quality management programs, and other initiatives as needed.
Contributes as a member of the interdisciplinary team during medical emergencies (e.g., Code Blue).
Performs other duties as required.

Basic Qualifications:
Experience


Must possess a minimum of 1200 hours of inpatient pharmacy experience within the last five years (e.g., intern pharmacist, pharmacist) or have successfully completed an ASHP accredited clinical pharmacy practice residency.

Education
Degree in Pharmacy that meets the minimum educational requirements for licensure.
License, Certification, Registration Controlled Substance Utilization Review and Evaluation System Registration required at hire.
Pharmacist License (California) required at hire.
National Provider Identifier required at hire.
Advanced Cardiac Life Support certification within three months of hire from the American Heart Association.
Basic Life Support certification within three months of hire from the American Heart Association.

Additional Requirements:


Demonstrates knowledge of patient variables such as age, gender, and other physical characteristics/presentation and disease states as they relate to medication therapy.

Comprehensive knowledge of acute care medications.

Possesses extensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, and disease states, and the ability to apply this information to the patient's medical management plan.

Demonstrated experience in performing aseptic techniques and the ability to prepare sterile IV products and chemotherapeutic agents.
Excellent verbal and written communication skills.
Knowledge of federal and state laws and regulations governing the provision of pharmaceutical services.
Successful completion of a standardized competency assessment OR current incumbency in job codes 60002, 60007, 67130, or 67150.
Demonstrated ability to extract relevant information from patients or other sources and relate it to the patient's medication management plan.
