As a Pharmacist within Kaiser Permanente, your primary responsibility will be to oversee and manage medication therapy for both hospitalized and outpatient individuals. This position requires close collaboration with healthcare professionals, adjusting medication dosages, ordering necessary laboratory tests, and administering medications as required. You will also be tasked with providing essential medication information, pharmacokinetic dosing recommendations, and guidance to pharmacy personnel.
Key Responsibilities:- Adhere to Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and relevant state, federal, and local regulations.
- Conduct both sterile and non-sterile compounding.
- Formulate and execute patient-specific strategies to enhance medication therapy.
- Evaluate medication therapy to ensure optimal outcomes.
- Manage and refine medication therapy based on patient-specific data.
- Interview patients to gather information regarding their medication usage and allergies.
- Provide counsel to patients on the proper use and storage of medications.
- Deliver pharmacokinetic dosing guidance to healthcare providers.
- Supervise medication distribution and confirm accuracy.
- Identify potential medication safety concerns and implement preventative measures.
- Engage in educational initiatives for pharmacy and medical staff.
- Foster a collaborative work environment and report discrepancies as necessary.
- At least 1200 hours of inpatient pharmacy experience within the past five years.
- Pharmacy degree that meets licensure criteria.
- Possession of required licenses and certifications, including Controlled Substance Utilization Review and Evaluation System Registration, Pharmacist License (California), National Provider Identifier, and Basic/Advanced Life Support certifications.
- Demonstrated understanding of patient variables and disease states relevant to medication therapy.
- Thorough knowledge of acute care medications and pharmacological principles.
- Experience with aseptic techniques and sterile IV preparation.
- Strong communication skills and familiarity with pharmacy regulations.
- Ability to extract and apply information to develop patient treatment plans.
- Proficiency in pharmacy information systems and emergency medications.
- Successful completion of competency assessments or relevant professional experience.
- Doctorate of Pharmacy (Pharm.D.)
