RN, Case Manager, Home Health

Job Summary:

Sutter Health is seeking an experienced RN to fill the role of Case Manager, Home Health. As a key member of our interdisciplinary team, you will be responsible for managing patient care services, ensuring the optimal degree of quality care is maintained. This role requires strong clinical assessment skills, effective communication, and the ability to work independently and as part of a team.

Responsibilities:
  • Plan, organize, and direct patient care services for patients in the assigned caseload
  • Coordinate the interdisciplinary team to implement the established plan of patient care
  • Ensure effective management of visit utilization and control of expenses
  • Assist in agency-wide orientation, competency evaluations, and consultation to clinical staff related to specific expertise
Requirements:
  • Graduate of an accredited School of Nursing
  • RN-Registered Nurse of California
  • BLS-Basic Life Support Healthcare Provider
  • DL-Valid Drivers License Class C
  • AUTO-Automobile Insurance
  • 2 years recent relevant experience
Preferred Qualifications:
  • Ability to function independently and as a member of an interdisciplinary team
  • Ability to exercise independent, sound nursing judgment in planning and providing patient care
  • Exhibits knowledge of patho-physiology and accepted treatment protocols for common home care diagnoses
  • Knowledge of basic safety and infection control principles

Pay Range:

$68.43 to $91.00 per hour

The salary range for this role may vary above or below the posted range as determined by location. Compensation takes into account several factors, including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs.
