About the Role:

As a Case Manager RN, you will provide highly skilled nursing care in the home setting, admitting patients and providing services such as wound care, IV medication administration, and supervising LVN care delivery and care coordination with the multidisciplinary team. You will be the front-line clinician, communicating with the physician and overseeing the plan of care.

Responsibilities:
  • Organize, communicate, and align a multidisciplinary team to create an inclusive and collaborative team model.
  • Provide highly skilled nursing care in the home setting, including wound care, IV medication administration, and supervising LVN care delivery.
  • Communicate with the physician and oversee the plan of care.
  • Lead the case conference and improve clinical outcomes.
Requirements:
  • California Registered Nurse license.
  • Current AHA BLS certification.
  • California driver's license with proof of liability insurance for automobile.
  • Preferred: One year acute care experience as a RN, Home Health experience preferred.
