Role Overview: The HCBS Hospital Liaison will work closely with the hospital case management team to facilitate a seamless transition for patients moving from hospital care to community-based services. This position is essential in linking patients to Home and Community Based services while addressing their complex needs.
Key Responsibilities:
- Connect individuals to Home and Community Based service programs
- Engage in hospital meetings and contribute to care planning
- Comply with hospital policies and maintain patient confidentiality
- Coordinate resources to support patient discharge
- Collaborate with healthcare professionals and community organizations
- Evaluate patient needs and determine eligibility for services
- Conduct follow-ups with patients after discharge
- Document all interactions and interventions accurately
- Provide training to hospital staff regarding community care options
Qualifications: A Bachelor’s degree in social work or a related field is required, with a preference for experience working with older adults. A valid driver’s license and reliable transportation are necessary. Strong interpersonal and problem-solving abilities are essential, along with the capacity to thrive in a team environment.
Work Hours: 37.5 hours per week
