Patient Care Coordinator

This is a challenging and rewarding role that requires a skilled and compassionate individual to provide comprehensive care coordination to patients. As a Patient Care Coordinator at Fairview Health Services, you will be responsible for assessing patient needs, developing and implementing care plans, and ensuring seamless transitions across the healthcare continuum.
Key Responsibilities- Manage patient care across the healthcare continuum to achieve efficient clinical, financial, and operational outcomes.
- Act as a single point of contact for patients, physicians, and care providers throughout the patient's hospitalization.
- Initiate and implement transition functions and activities for patients, communicating with patients, families, and the healthcare team to ensure seamless transitions.
- Assess and document learning needs of patients and families, developing individualized education plans based on identified needs.
- Establish and implement nursing care plans for assigned patient populations through assessment, development, activation, evaluation, and discharge.
- Collaborate with healthcare team partners and patients/families to manage patient discharge plans.
- Optimally communicate the plan across the continuum of care.
- Assist in the development and implementation of process improvement activities to achieve efficient clinical, financial, and satisfaction outcomes.
- Collect data and other information required by payers to fulfill utilization and regulatory requirements.
- Identify and communicate issues related to case escalation to appropriate leaders.
- Establish a collaborative relationship with physicians, medical directors, nurses, and other unit staff, and payers.
- Mentor internal members of the healthcare team on case management and managed care concepts.
- Understand and focus on key performance indicators, actively supervising outcomes and participating in quality planning.
- AD in Nursing
- 5 years clinical experience
- 1+ years working as a care coordinator/case manager
- Active MN Registered Nurse license
- Preferred: Case Management Certification
