Responsibilities- Provide one-on-one care to pediatric patients in their homes, focusing on their unique needs and requirements.
- Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure seamless care coordination.
- Administer medications, perform medical procedures, and provide wound care as needed.
- Monitor patients' conditions, reporting any changes or concerns to healthcare teams and families.
- Collaborate with healthcare teams to ensure continuity of care, sharing knowledge and expertise to optimize patient outcomes.
- RN/LPN with a current valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
- Experience working with pediatric patients, preferably in a home health setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently, with minimal supervision, and as part of a team.
