About the Role:As a Pediatric Home Health Nurse, you will provide high-quality clinical care to medically fragile children in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that promote the health and well-being of our patients.
Responsibilities:- Provide skilled nursing care to pediatric patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans to address these needs.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination of services.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and to address any concerns or questions.
- Document patient care and progress in accordance with company policies and regulatory requirements.
- RN/LVN with a current valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
