We are seeking a skilled Licensed Practical Nurse to join our team at Penn Center Residential Care Facility. As a member of our interdisciplinary team, you will provide high-quality care to individuals working towards recovery from mental health conditions.
Key Responsibilities- Conduct comprehensive assessments of clients' mental health needs and develop individualized care plans.
- Collaborate with psychiatric providers, care coordinators, and other team members to ensure seamless care delivery.
- Provide education and support to clients and their families on mental health conditions, treatment options, and recovery strategies.
- Participate in multidisciplinary team meetings to discuss client progress and develop treatment plans.
- Iowa LPN licensure required.
- Experience working with individuals with mental health conditions and/or substance use disorders preferred.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
