Nurse Practitioner

We are searching for a skilled and compassionate Nurse Practitioner to join our memory care team. As a key member of our interdisciplinary team, you will work closely with other healthcare professionals to provide high-quality care to our residents.
Your responsibilities will include conducting assessments, developing treatment plans, and collaborating with physicians to ensure optimal resident outcomes. You will also be involved in educating family members and caregivers about dementia-related issues and providing guidance on support strategies.

Responsibilities:

- Provide education and support to family members and caregivers on dementia-related topics, promoting informed decision-making and self-care
- Work closely with our interdisciplinary team to deliver person-centered care, prioritizing resident comfort, dignity, and quality of life
