About the Role:

We are looking for a compassionate and experienced Medical Social Worker to provide medical social services to patients and their families. As a Medical Social Worker, you will work under the direction of a physician and Interdisciplinary Group to assess the social and emotional needs of patients and develop coping mechanisms.

Responsibilities:
  • Assess the social and emotional needs of patients and develop a comprehensive plan of care.
  • Provide medical social services to patients and their families, including counseling, education, and support.
  • Collaborate with the healthcare team to develop and implement a plan of care that addresses the patient's physical, emotional, and social needs.
  • Provide bereavement support and counseling to patients and their families.
  • Conduct home visits and provide medical social services to patients in their homes.
Requirements:
  • Master's Degree in Social Work from a Council of Social Work Education-accredited program.
  • One year of experience in a healthcare setting.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • OR: Must be licensed as a Social Worker from the State Board of Social Work.
