Housing Navigator

Housing Navigator Job Description
The Housing Navigator will require a compassionate individual dedicated to supporting and empowering individuals facing homelessness within our service area. The ideal candidate will have a strong understanding of social services and be able to provide guidance and resources to those in need.
Key Responsibilities:
- Coordinate with local agencies to address the needs of households
- Perform screenings, intakes, and assessments via Coordinated Entry System referrals
- Document services in HMIS/ServicePoint
- Direct clients to specialized services and resources
- Promote permanent housing opportunities by engaging and developing relationships with property owners/managers
Requirements:
- College education, with emphasis in social work or human services, is desired
- High School diploma or equivalent
- Valid driver's license for traveling to each of the counties
- Family Development Specialist, ROMA (Results Oriented Management Accountability), Community Health Care Worker Training, ServicePoint HMIS (Homeless Management Information System)
- One year experience working in social service programs with case management experience preferred
Benefits:
