Our care team consists of physicians, advanced practice professionals, medical assistants, behavioral health support, quality-based coders, referral coordinators, and more.

Job Summary

The Cardiologist will work closely with our team-based care model to support our value-based care provider focused on quality of care for the patients we serve. The ideal candidate will have a strong background in cardiology, excellent communication skills, and a passion for delivering high-quality patient care.

Responsibilities
  • Evaluate and treat center patients in accordance with standards of care.
  • Assist in the coordination of patient services, including specialty referrals when appropriate.
  • Act as an active participant and key source of medical expertise with the care team.
  • Partner with primary care physicians and nurse practitioners in a tone of cooperation in practice.
  • Complete all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation.
  • Follow policy and protocol defined by Clinical Leadership.
  • Meet with primary care physicians and market medical director to discuss quality of care, review of outcome data, and medical records issues.
Qualifications
  • Graduate of an accredited MD or DO program.
  • Current and unrestricted medical license or willing to obtain a medical license in the state of practice.
  • Board Certification in Cardiology.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • Medicaid Provider Number.
  • Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
  • Fully engaged in the concept of team-based care model.
  • Willingness and ability to learn/adapt to practice in a value-based care setting.
