Job Summary

We're seeking a skilled Cardiologist to join our team-based care model. As a Cardiologist, you'll work closely with our primary care physicians and nurse practitioners to provide comprehensive care to our patients. Your expertise will be invaluable in helping us achieve our goal of delivering the highest quality care to our patients.

Responsibilities
  • Evaluate and treat patients with cardiovascular conditions, including heart failure, coronary artery disease, and stroke.
  • Collaborate with our care team to develop and implement individualized care plans for patients.
  • Participate in quality improvement initiatives to enhance patient outcomes and reduce readmissions.
  • Stay up-to-date with the latest medical research and guidelines to ensure the highest level of care for our patients.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Board certification in Cardiology.
  • Current and unrestricted medical license.
  • Active DEA license.
  • Medicare and Medicaid provider numbers.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team-based care model.
Preferred Qualifications
  • Experience working in value-based care models.
  • Knowledge of HEDIS/STARS and CAHPS/HOS metrics.
  • Experience working with electronic health records.
About the Role
