About the Role

As a Medical Care Coordinator with Eliot Community Human Services, you will play a vital role in ensuring that our clients receive comprehensive care management services.

We are seeking an experienced Registered Nurse (RN) to join our interdisciplinary team in coordinating medical, behavioral health, and social needs of our clients.

About the Team

You will work closely with our team to provide care coordination services, interventions, and education to our clients. Your strong commitment to mental health recovery and substance abuse disorders will be an asset in this role.

About the Program

Eliot's One Care Health Home program is an innovative care management approach for members with complex medical, behavioral health, and social needs who are dual eligible. As a Medical Care Coordinator, you will have the opportunity to make a meaningful impact on the lives of our clients.

Responsibilities
  • Responsible for annual comprehensive assessments and supplements
  • Provide insight and guidance regarding client person-centered care plans
  • Coordinate authorizations and service referrals with Commonwealth Care Alliance (CCA)
  • Document care coordination activities and data in CCA's electronic platforms
  • Provide education, consultation, and care coordination to active enrollees
Qualifications
  • Licensed Registered Nurse with at least two years of experience working with people with severe and persistent mental illnesses or at least two years of providing medical coordination
  • Skills to establish supportive relationships with persons with severe and persistent mental illnesses and respect for their preferences for treatment
  • Ability to work on a team that provides care coordination services and interventions
Benefits
  • $75,000 annual salary
  • Monday-Friday schedule from 9am-5pm
  • Service areas include Lowell, Lawrence, Lynn, North Shore, Everett, and Malden offices
