Nurse Care Coordinator

Mass General Brigham is seeking a skilled Nurse Care Coordinator to join our Home Hospital team in Everett, Massachusetts.
The ideal candidate will have a strong background in nursing and excellent communication skills to coordinate patient care across various settings.
Responsibilities- Develop and implement personalized care plans that meet the unique needs of patients.
- Collaborate with healthcare professionals to ensure seamless coordination of care.
- Provide education and support to patients and their families to promote healthy outcomes.
- Maintain accurate and up-to-date records of patient information and progress.
- Bachelor's degree in Nursing (BSN) or equivalent.
- Current RN licensure in Massachusetts.
- Minimum one year of experience in a healthcare setting, preferably in home health or case management.
- Excellent communication and organizational skills.
- Ability to work independently and as part of a team.
- Competitive salary: $105,000 - $120,000 per year.
