Nurse Manager

VNS Health is seeking a skilled Nurse Manager to oversee the day-to-day activities of our specialized programs and products. As a key member of our team, you will be responsible for ensuring the integration of evidence-based care practices into protocols, policies, and continuous quality improvement initiatives.
Key Responsibilities- Program Management: Manage the day-to-day activities of one or more of our specialized programs or products, ensuring effective workflows and protocols are in place.
- Care Coordination: Act as a liaison for care management teams to ensure our programs are meeting expected outcomes, implementing changes as necessary.
- Clinical Leadership: Provide clinical leadership and guidance to staff, promoting staff understanding of tele-management processes and their value for patients, medical providers, and healthcare partners.
- Patient Education: Assess, educate, and improve patient knowledge of chronic disease, self-care management, and identification of changes in health status, including appropriate responses and actions through individualized education and multifaceted interventions.
- Quality Improvement: Review VNS Health patient records for cases that were readmitted during an active home care episode, leading and coordinating root cause analysis and developing recommendations for quality improvement measures.
- Operational Oversight: Review productivity reports, analyze trends, and implement corrective measures to address performance or operational issues.
- Team Management: Conduct team audits, assist staff in navigating patient care, and oversee metric reporting, working with Business Operations to create weekly departmental KPI reports.
- Senior Leadership Support: Assist senior leadership with development of VNS Health client outcomes reporting and other analyses of clinical data and quality reporting as needed.
- Licensure and Certification: License and current registration to practice as a Registered Professional Nurse in NYS, with Care Management or Case Management certification within one year of job entry date.
- Education: Relevant degree needed for professional licensure, with a Master's Degree in a healthcare-related field preferred.
- Experience: Minimum of three years of clinical experience, with experience in case management, administration, or discharge planning in a hospital setting preferred.
- Skills: Exceptional customer service skills, demonstrated ability to engage clinical counterparts in collaborative discussions, strong follow-up skills, and ability to manage multiple priorities.
