Key Responsibilities
  • Coordinate and oversee the coordination of benefits and services for patients on your caseload.
  • Complete comprehensive care management and disease-specific assessments to identify patient needs and develop personalized care plans.
  • Maintain timely telephonic contact with patients to monitor their progress and address any concerns or issues that may arise.
  • Resolve complex issues and member complaints impacting the delivery of services, ensuring that patients receive the care they need in a timely and efficient manner.
  • Provide health education and support to patients and their caregivers, empowering them to take an active role in their healthcare.
  • Identify and address social determinants of health that may impact patient care, working to remove barriers to care and ensure that patients receive the support they need to achieve optimal health outcomes.
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination and effective communication.
  • Advocate for patients and their caregivers, ensuring that their needs are met and their voices are heard.
  • Stay up-to-date with the latest evidence-based clinical practice guidelines and best practices in care management, applying this knowledge to inform your work and improve patient outcomes.
Requirements
  • Current licensure and registration as a Registered Professional Nurse in New York State.
  • Certified Case Manager (CCM) certification within one year of employment.
  • Associate's or Bachelor's degree in nursing or a related field.
  • Minimum two years of experience in a healthcare-related field, with a focus on care management and/or managed care.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, caregivers, and healthcare providers.
