Job Title: RN Care Coordinator

St. Elizabeth Healthcare is seeking a skilled RN Care Coordinator to join our team. As a RN Care Coordinator, you will work collaboratively with providers, interdisciplinary staff, and clinical associates to support patients with chronic conditions and/or complex needs.

Job Summary:

The RN Care Coordinator will facilitate effective communication, coordinate services, address barriers, and provide education and guidance for patients related to current health concerns. This role requires a strong understanding of anatomy and physiology, pharmacology, and ambulatory care management.

Responsibilities:
  • Collaborate with patients, providers, and interdisciplinary staff to assess needs and develop a patient education plan of care.
  • Provide education and pre-printed, SEP-approved educational materials as needed, or at provider or patient request.
  • Work collaboratively with patients to identify and address barriers to care.
  • Facilitate effective communication and coordination of services among healthcare providers and teams.
  • Document patient information accurately and completely in the Electronic Health Record.
Requirements:
  • Current State Allied Health License required, RN.
  • Current Driver's License in good standing and reliable and insured transportation.
  • Degree in nursing or other related clinical field.
  • Care Management Certification preferred.
Preferred Qualifications:
  • Three to four years medical field experience.
  • Ambulatory and/or care management experience preferred.
