Senior Community Care Clinician Opportunity

As a Senior Community Care Clinician, you will be part of an interdisciplinary team that coordinates the delivery and provision of clinical care to our highest-risk health plan members.

About the Role

This is a travel-based position requiring willingness to travel within Oregon and Washington. The role involves 100% travel throughout these states, providing care to senior patients in skilled nursing facilities. You will work closely with the primary Advanced Practice Clinician (APC) during periods of absence, ensuring seamless care for our members.

Responsibilities
  • Provide clinical coverage for the primary APC during periods of PTO or short/long-term absences
