About the Role

Nascentia Health is seeking a highly skilled Registered Nurse, Case Manager to join our team of healthcare professionals. As a key member of our home health services team, you will be responsible for providing direct patient care, coordinating overall home care services, and documenting care for agency clients in accordance with Quality Standards.

Key Responsibilities
  • Ensure quality and safe delivery of home health services, including case managing client caseloads, developing plans of care based on client assessments and OASIS evaluations, and ensuring all documentation meets quality standards.
  • Complete all OASIS evaluations, documentation reflects nursing care and interactions, and documentation is clear, concise, and completed in a timely manner.
  • Coordinate LPN activities, provide guidance and education on client care, and work with the Clinical Supervisor and Patient Care Coordinator to efficiently manage patients using a team approach.
  • Supervise paraprofessional staff, including home health aides, in performing designated tasks related to patient care and communicating relevant information to the home health aide department.
  • Comply with all Nascentia Health policies and procedures, maintain patient and employee confidentiality, and demonstrate knowledge and use of infection control, federal, state, and local government rules and regulations.
Requirements
  • Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.
  • Current and valid driver's license and auto insurance, and a vehicle that can be used for work.
  • Self-directed and organized with the ability to work independently in a fast-paced environment.
  • Previous home care or case management experience a plus.
  • Proficient computer skills, including knowledge of electronic medical records software, with the ability to learn new software.
  • Exceptional oral, written, and verbal communication skills.
Physical Requirements
  • Ability to sit or stand at a desk 90% of the day.
  • Ability to perform tasks involving physical activity, including pushing/pulling, lifting/carrying, patient transfers, and extensive bending, standing, kneeling, and walking in various weather conditions.
  • Ability to drive in all weather conditions.
  • Visual and hearing ability sufficient to comprehend written or verbal communication.
  • Ability to use a telephone and communicate over the phone.
Compensation and Benefits
