About the Role

We are seeking a skilled Transplant Coordinator- LPN to join our team at Emory Healthcare's Kidney Transplant Program. As a key member of our outpatient team, you will focus on the overall health and population management of a cohort of patients.

Key Responsibilities
  • Triage in basket and pool messages per clinic specified guidelines and security settings
  • Review results per clinic specified guidelines and security settings
  • Coordinate care across settings
  • Facilitate care management of defined populations
