Job Summary

Key Responsibilities
  • Provide nursing care to assigned patients, including administering medications, performing treatments, and documenting patient information.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
  • Contribute to a positive and supportive team environment, promoting a culture of excellence and compassion.
Requirements
  • Tennessee LPN license or Licensed Vocational Nurse
  • One year of professional experience in hospital or prior home health experience
  • Successful completion of LPN advanced skills course
  • CPR certification
  • Valid Tennessee driver's license and state-mandated minimum insurance coverage
