Job Summary

We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Home Health Registered Nurse. As a key member of our care team, you will be responsible for providing high-quality patient care and support to individuals in their homes. Key Responsibilities
  • Provide comprehensive patient care and support in the home setting, including assessments, care planning, and implementation of care plans.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Develop and maintain strong relationships with patients and their families, providing emotional support and education on health and wellness.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
  • Contribute to the development and implementation of care policies and procedures to ensure high-quality patient care.
Requirements
  • RN license in the state of employment.
  • Graduate from an approved school of nursing.
  • One year of RN experience and clinical competence to deliver quality patient care.
  • Current driver's license and ability to spend approximately 20% of work time driving to/from patient locations.
  • Commitment to consistently meet critical deadlines for charting.
  • Ability to self-manage time and schedule.
  • Demonstrated experience with tablets, mobile phones, and EMR software.
