Key Responsibilities:- Provide professional nursing services under the direction of a RN, adhering to state regulations, healthcare standards, and CenterWell Home Health policies.
- Develop and implement individualized care plans, focusing on problem-specific therapeutic interventions, teaching, and training activities.
- Collaborate with the RN Supervisor to ensure seamless care coordination and make informed decisions about patient care.
- Communicate effectively with patients, physicians, and healthcare teams to ensure timely and efficient care delivery.
- Participate in care coordination activities, discharge planning, and maintain confidentiality of sensitive patient information.
- Ensure adherence to professional standards, maintain accurate documentation, and prepare reports as required.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
