Key Responsibilities- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitative, preventative, and restorative care.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication with patients, families, and healthcare providers.
- Conduct thorough assessments, identify patient needs, and develop strategies to address these needs in a timely and effective manner.
- Document patient progress, outcomes, and any changes in their condition, ensuring accurate and comprehensive records.
- Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
- Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
