Position Overview
The Clinical Nurse Specialist for Wound Care and Continence plays a pivotal role in delivering specialized nursing care under the guidance of the Nurse Manager. This position focuses on providing acute and rehabilitative support for patients with specific disorders affecting the gastrointestinal, genitourinary, and integumentary systems. The specialist serves as a key resource for both nursing and medical teams, ensuring comprehensive patient care in complex clinical scenarios.
The Clinical Nurse Specialist is a licensed professional who practices with competence, advocates for patient needs, adheres to organizational policies, and strives for excellence in service delivery within a collaborative governance framework. Clinical practice is informed by evidence-based approaches, ongoing education, and reflective practice. This role anticipates and addresses the care requirements of patients, their families, and significant others.
Key Responsibilities and Functions
Expert Practice and Patient Outcomes
- Consults with physicians, assessing patients and developing tailored, evidence-based care plans addressing wounds, ostomies, and continence challenges.
- Responds to physician requests by issuing orders within the scope of practice and guiding other nursing staff on care protocols.
- Marks patients for surgical procedures, providing pre- and post-operative education and support for psychosocial adjustments.
- Employs the Nursing Process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) to deliver individualized care, ensuring coordination through effective partnerships.
- Delivers patient-centered care by collaborating with patients and their families, utilizing comprehensive communication strategies and interdisciplinary evaluations.
- Maintains current clinical standards with a focus on safety and quality.
- In the outpatient Wound Care and Hyperbaric Unit, demonstrates knowledge of wound healing strategies and hyperbaric oxygen therapy protocols.
- Monitors patients during treatments, ensuring safety and responding to any adverse reactions.
- Documents all aspects of patient care clearly and concisely, including treatment settings and patient responses.
- Ensures proper operation of hyperbaric chambers and maintains accurate logs of chamber activities.
- Participates in clinical committees and community organizations, leading evaluations and improvement initiatives related to wound care.
- Promotes interdisciplinary communication and planning to ensure safe, quality patient care.
- Acts as a mentor for nursing staff and students, fostering a qualified workforce and a supportive work environment.
- Engages in evidence-based practice and research to enhance nursing care and patient outcomes.
- Collaborates in the development of protocols and procedures to achieve best practice outcomes.
Education: Graduate of an accredited nursing program with a preference for a Baccalaureate degree in nursing. Completion of a specialized Wound, Ostomy, and Continence nursing program is preferred.
Licenses and Certifications: Must hold a valid nursing license and certifications in Basic Life Support and any relevant specialty areas.
Experience: A minimum of three years in an acute care nursing role is required.
Skills: Strong communication and interpersonal skills, ability to function in a fast-paced environment, and a commitment to evidence-based nursing practice are essential for success in this role.
