We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Case Manager in our Hospice Care department. As a key member of our care team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities- Perform patient assessments and develop comprehensive care plans in collaboration with the care team.
- Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
- Develop, prepare, and maintain accurate and timely patient care progress records.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with clinical managers and care team members.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- One year of nursing experience.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
