We are seeking a skilled Health Coach to join our Care Management team. As a Health Coach, you will play a vital role in promoting wellness and preventative care, improving member quality of care and health outcomes, reducing hospitalization, and ensuring compliance with regulatory requirements.
Key Responsibilities:
- Perform in-person care management visits to assess patients' needs and provide guidance on healthy living.
- Assist with benefit coordination, order routine supplies, and troubleshoot delivery issues.
- Notify clinical staff of identified issues and clinical triggers requiring escalation.
- Complete patient assessments and provide feedback to clinical staff.
Requirements:
- Driver's license or government-issued ID
- Copy of Social Security card
- Application
- Medical history, including vaccination history
- Voided check
Training and Support:
We provide one-day training, which includes infection control training, paperwork/software training, and specific test training. Our team is committed to supporting you in your role as a Health Coach.
Compensation:
You will receive a fee for service, with compensation ranging from $40 to $70 per patient, depending on the number of tests administered.
